Abstract:To investigate the effect of surgical treatment of hepatic echinococcal cysts adjacent to hepatic porta. We retrospectively analyzed the surgical treatment(Total subadventitial cystectomy, part subadventitial cystectomy and endocystectomy ) of 36 parahilar hydatid cysts in 20 patients from November 2015 to January 2019. A total of 4 hydatid cysts were successfully treated with the operation "total subadventitial cystectomy". 30 hydatid cysts were successfully treated with the operation "part subadventitial cystectomy". Because the outer cyst adhered too closely to the hepatic hilar vessels and had been ruptured, 2 hydatid cysts were treated with the operation "endocystectomy". For the surgical treatment of hepatic echinococcosis adjacent to hepatic hilum, total subadventitial cystectomy and part subadventitial cystectomy are preferable to traditional endocystectomy, and the incidence of complications such as residual cavity infection and biliary leakage are relatively low. However, specific surgical methods should be chosen according to the location, size and intraoperative specific conditions of the cyst.
