Applied research on T-SPOT.TB for the diagnosis of renal tuberculosis |

Abstract In this study, we evaluated the diagnostic value of T-SPOT.TB for renal tuberculosis. For this research, T-SPOT.TB and TST were administered respectively for the diagnosis of renal tuberculosis among 112 patients with suspected renal tuberculosis. The data were analyzed by Kappa test and z test. Our results showed that of 112 patients, there were 44 cases of renal tuberculosis by clinical diagnosis. With clinical diagnosis as a reference standard, the sensitivity, specificity, agreement, Youden index, and Kappa value with T-SPOT.TB were 95.5%, 92.6%, 93.8%, 0.881, and 0.871, respectively; while with TST, the sensitivity, specificity, agreement, Youden index, and Kappa value were 97.7%, 62.3%, 75.0%, 0.60, and 0.525, respectively. The sensitivity and negative predictive value of T-SPOT.TB and TST were very similar and no statistically significant difference was observed between them (P>0.05). The specificity and positive predictive value of T-SPOT.TB were higher than those of TST and the significant differences was observed between them (P≤0.05). Hence, T-SPOT.TB is superior to TST for the detection of renal tuberculosis and could be used to assist the diagnosis of renal tuberculosis.
